Below is a list items that we need to be able to properly process your order. You may have discussed these items with your representative. It is helpful if you forward answers to these items to our team to ensure the quickest and most accurate order process.

1. Letter of Agency – The purpose of this form is to provide authorization to Revonix to work with your existing phone provider during the transition to the new phone system. If you are not keeping existing phone number, this is not required.

2. Phone number Portability – The purpose of this form is to identify phone number(s) you desire to keep and authorizes us to port to Revonix (Level3 – our porting agent) from your existing phone company. If you are not keeping any existing phone numbers, this does not apply. Just let us know.

3. 800 Number Portability – The purpose of this form is to identify 800/888/866 phone number(s) you desire to keep and authorizes us to port to Revonix (Level3 – our porting agent) from your existing phone company. If you are not keeping any existing 800 phone numbers, this does not apply. Just let us know.

4. Fax Portability – The purpose of this form is to identify Fax phone number(s) you desire to keep and authorizes us to port to Revonix (J2 – our porting agent) from your existing phone company. Again, if you are not keeping or moving your fax service to us, this does not apply. Just let us know.

5. Please fill out your extension number with names and related Phone Setup Information.

6. Tell us about how your phone are answered today.  Call Flow

7. Please fill out the Network Readiness Document – to the best of your ability. It is usually easiest if your IT staff fills this out.

8. A copy of all current phone bills needs to be sent to Revonix. This is necessary to ensure quick phone number porting from your existing provider to Revonix.
